The privacy and security of the personal information we maintain is of the utmost importance to Paradigm Healthcare Services (“Paradigm”). Paradigm is a third-party service provider that processes Medi-Cal (California’s Medicaid program) billing and maintains Medi-Cal member identification numbers on behalf of and pursuant to a Data Use Agreement with the California Department of Health Care Services (“DHCS”). Please read this notice carefully, as it provides information about the incident and the significant measures we take to protect your information.

What Happened?

On October 13, 2025, Paradigm became aware of unauthorized access to its local network. Paradigm’s online service documentation and billing platform, Student Health Network (“SHN”), was not impacted by this incident. 

What We Are Doing.

Immediately upon detecting this activity, we took steps to mitigate the incident, including taking certain systems offline and notifying law enforcement. We also quickly engaged professionals experienced in handling these types of incidents to conduct an investigation to determine what, if any, information was impacted. On August 18, 2026, we determined that certain files containing select individuals’ protected health information may have been accessed by an unauthorized party between October 8, 2025, and October 15, 2025.  

What Information Was Involved?

The information that may have been accessed included your name, date of birth, gender, and Medi-Cal member identification number. 

What You Can Do.

We have no evidence that any of your information has been used to commit fraud or identity theft as a result of this incident. Nevertheless, out of an abundance of caution, we want to make you aware of the incident. This notice provides precautionary measures you can take to protect your personal information, including placing a Fraud Alert and Security Freeze on your credit files, and obtaining a free credit report. Additionally, you should always remain vigilant in reviewing your financial account statements and credit reports for fraudulent or irregular activity.
